چکیده

Classic approaches to the head and neck are well described in the surgical literature. These “external” or “open” approaches correspond to traditional anatomic dissections and are thus very familiar to head and neck surgeons. In contrast, the anatomy of the larynx and pharynx from a transoral robotic (TORS) standpoint must be approached from an “inside-out” perspective. Thorough knowledge of this anatomic perspective is necessary for achieving appropriate oncologic resection as well as avoiding potentially catastrophic complications. The transoral approach to the pharynx and larynx has been described in the past, especially in the setting of transoral laser microsurgery (TLM). Wolfgang Steiner and colleagues have described several such approaches and procedures [ 1 ]. Although laryngoscopic exposure is considerably different than that achieved with the TORS approach, many of the anatomic details and considerations overlap. Anatomic considerations of the TORS approach can be viewed from a perspective of (1) myofascial layers of the surgical fi eld and (2) neurovascular structures that traverse the fi eld. Given the confi ned space of dissection, threedimensional understanding is critical. In addition, familiarity with this anatomic region of the head and neck from a trans-cervical standpoint is important for understanding the TORS perspective.

The First Successful Case of Transoral Robotic Surgery in a Patient with Sialadenoma Papilliferum

Introduction: Sialadenoma papilliferum (SP) is a rare benign tumor, which originates from the minor salivary gland. It occurs at sites that have minor salivary glands, such as the palate, retromolar pads, buccal mucosa, and lips. The most common location for tumor development is on the hard palate. A differential diagnosis consists of ruling out other salivary gland tumors. Transoral robotic s...

Surgical anatomy of the supraglottic larynx using the da Vinci robot.

BACKGROUND Transoral robotic surgery (TORS) has facilitated organ-preserving surgery of the larynx. It has also presented a change in the surgical perspective. We performed cadaveric dissections using the robot to highlight the vascular and muscular anatomy of the supraglottic larynx. METHODS Cadaveric specimens underwent injection of their vasculature, and after injection a robotic surgical ...

Transoral robotic surgery in the management of head and neck tumours

The article reviews the use of robotic technology for head and neck tumours. The authors discuss the development of transoral robotic surgery (TORS), the current status of the technology, and the set-up in the operating room. The article provides a review of the literature, highlighting the applications, advantages, functional outcomes, and disadvantages of TORS for each anatomic subsite (oroph...

Flex Robotic System in transoral robotic surgery: The first 40 patients.

BACKGROUND The Flex Robotic System is a new robotic device specifically developed for transoral robotic surgery (TORS). METHODS We performed a prospective clinical study, assessing the safety and efficacy of the Medrobotics Flex Robotic System. A total of 40 patients required a surgical procedure for benign lesions (n = 30) or T1 and T2 carcinomas (n = 10). Access and visualization of differe...

Transoral robotic total laryngectomy: report of 3 cases.

BACKGROUND Because of the significant toxicity of chemoradiation regimens, there has been a resurgence of interest in the primary surgical management of head and neck cancer and, in particular, the use of minimally invasive surgery. One such technique is transoral robotic surgery (TORS). We aim to discuss the potential role of TORS in patients requiring total laryngectomy (TL). METHODS AND RE...
